Presgripsiynau Amlroddadwy    Repeat Prescriptions

Yn debyg i'r holl feddygfeydd cyffredinol, rydym yn cynnig gwasanaeth presgripsiynau amlroddadwy.

Mae llwyddiant y cyfleuster hwn yn dibynnu ar ei gynnal mewn modd manwl iawn. Caiff presgripsiynau eu creu gan gyfrifiadur ond mae angen i ni oruchwylio manwl-gywirdeb y presgripsiwn yn gyson.

Er mwyn lleihau camgymeriadau, byddwn yn derbyn ceisiadau am bresgripsiynau'n ysgrifenedig yn unig, a hynny ar y slip presgripsiynau amlroddadwy a ddarperir. Yna bydd eich presgripsiwn yn barod i'w gasglu o'r feddygfa ar ôl 2pm, dau ddiwrnod yn ddiweddarach, ac eithrio ar ddydd Sadwrn, ar yr amod eich bod yn cadw at y camau syml hyn. Cofiwch nad yw ein staff yn gweithio Gwyliau'r Banc. Ni fydd presgripsiwn sy'n cael ei archebu dros y rhyngrwyd yn ystod y cyfnod hwn yn barod tan ar ôl 2.00pm ar y diwrnod y byddwn yn ailagor. Os bydd angen i chi ail-archebu'n gynnar, nodwch y rheswm pam neu mae'n bosibl y caiff eich cais ei wrthod gan y meddyg.

Bob hyn a hyn, bydd gofyn i chi drefnu apwyntiad i weld y meddyg er mwyn sicrhau bod modd gwirio'r cyffuriau sy'n cael eu rhagnodi ac i addasu'r slip amlroddadwy os bydd angen.

Mae rhai o'r siopau fferyllydd lleol yn cynnig gwasanaeth casglu a chludo i bobl sy'n gaeth i'r tŷ ac nad ydynt yn gallu symud. Ni allwn drefnu hyn ar eich rhan - mae angen i chi gysylltu â'r siop fferyllydd o'ch dewis ac i lenwi'r ffurflen y byddant yn ei rhoi i chi.

Dylech fod yn ymwybodol pa bryd y bydd eich meddyginiaeth yn dod i ben a gadael digon o amser i gael presgripsiwn amlroddadwy - gall fod yn beryglus i roi'r gorau i'ch meddyginiaeth yn ddisymwth.

Like all general practice surgeries we offer a repeat prescription service. 

The success of this facility depends on running it very strictly. Prescriptions are generated by computer but the accuracy of the prescription requires constant supervision. 

In order to minimise errors we will only accept requests for repeat prescriptions in writing, preferably on the repeat slip provided. Your prescription will then be ready for collection from the surgery after 2pm, 2 days later, excluding Saturday, provided these simple steps are adhered to. Please remember our staff do not work Bank Holidays. A prescription ordered over the internet during this time will not be ready until after 2.00pm on the day we re-open.  If you need to re-order early please state the reason why or your request might be refused by the doctor.

Every so often you will be asked to make an appointment to see the doctor in order that the drugs prescribed can be checked and the repeat slip amended if necessary. 

Some of the local chemists offer a collection and delivery service for the housebound and immobile. We cannot arrange this for you - you need to contact the chemist of your choice and complete the form they provide. 

You should be aware of your medication running out and leave plenty of time to obtain a repeat prescription - it can be dangerous if you stop taking medication abruptly.
