Os bydd angen i chi weld meddyg penodol, byddwn yn ceisio cynnig apwyntiad wedi'i drefnu ymlaen llaw o fewn tair wythnos, ond oherwydd bod cleifion bellach yn cofrestru â'r practis yn hytrach na meddyg unigol, efallai na fydd bob amser yn bosibl i chi weld y meddyg o'ch dewis. Mae system galw heibio Gofal Brys ar waith i gleifion Bron Derw o 8.30am - 12.30 a 1.30 - 4.00pm i weld clinigwr ac nid meddyg teulu o'ch dewis. Gwasanaeth dan arweiniad nyrsys yw hwn. Caiff unrhyw glaf Bron Derw sy'n galw heibio ar ôl 4.00pm ei frysbennu gan glinigwr a fydd yn penderfynu p'un a oes angen iddynt aros i gael eu gweld a pha un a oes angen iddynt aros, dychwelyd y diwrnod canlynol neu p'un a fyddai'r fferyllydd yn un o'r siopau fferyllydd lleol yn gallu helpu. Pan fyddwch yn ffonio i gael apwyntiad wedi'i drefnu ymlaen llaw neu'n dod i gael Gofal Brys, byddem yn ddiolchgar pe baech yn gallu rhoi manylion cryno eich problem i'r derbynnydd fel bod modd i'r aelod mwyaf priodol o'n tîm eich gweld. Caiff ffurflen ar glipfwrdd ei chynnig i gleifion gofal brys ei llenwi er mwyn sicrhau cyfrinachedd llwyr wrth y ddesg. Mae dal angen apwyntiadau ar gyfer y Nyrsys a'r Cynorthwywyr Gofal Iechyd. Rydym yn ceisio cadw at amser gymaint â phosibl, ond yn anochel gall apwyntiadau redeg dros amser, felly byddwch yn amyneddgar os bydd yn rhaid i chi aros. I sicrhau bod y feddygfa yn rhedeg mor llyfn â phosibl, cofiwch y pwyntiau canlynol:

  • Dylech gyrraedd ar amser ar gyfer eich apwyntiad a chaniatáu amser i barcio eich car
  • Rhowch wybod i ni os na allwch gadw eich apwyntiad - byddwn yn ysgrifennu at gleifion nad ydynt yn dod i apwyntiadau ac ni fyddant yn cael trefnu apwyntiadau ymlaen llaw yn y dyfodol.
  • Peidiwch â thrio "ffitio i mewn" bobl eraill i'ch apwyntiad chi
  • Rhowch wybod i ni os oes angen mwy na'r 10 munud a ddyrennir fel bod modd i roi apwyntiad hwy i chi

Dylid gofyn am ymweliadau â'r cartref, lle bo'n berthnasol, mor gynnar yn y bore â phosibl. Mae hyn yn galluogi'r staff a'r meddyg teulu ar alwad i flaenoriaethu'r ymweliadau fel y caiff y rhai mwyaf brys eu gwneud gyntaf. Os byddwch yn hwyr i'ch apwyntiad, byddwn yn gofyn i'r clinigwr p'un a oes ganddo amser i'ch gweld. Os byddwch yn fwy na 10 munud yn hwyr, efallai y byddwn yn gofyn i chi aildrefnu eich apwyntiad.

If you need to see a named doctor we will try to offer you a pre-bookable appointment within the next 3 weeks, but as patients are now registered with the practice rather than with an individual doctor, it may not always be possible for you to see your preferred doctor. An Immediate Care walk in system is in place for Bron Derw patients from 8.30am - 12.30 and 1.30 -4.00pm to see a clinician not a GP of your choice. This is a nurse led service. Any Bron Derw patient walking in after 4.00pm will be triaged by a clinician who will make the decision as to whether they need to wait to be seen and will have to wait, come back the next day or whether the pharmacist in one of the local chemists could help.

When you telephone for a re-bookable appointment or arrive for Immediate Care we would be grateful if you would provide the receptionist with brief details of your problem so that you can be seen by the most appropriate member of our team. Immediate care patients will be offered a form on a clip board to complete to ensure complete confidentiality at the desk.

Appointments are still needed for the Nurses and Health care Assistants.

We try to run to time as far as possible, but inevitably an appointment can run over time, so please bear with us if you have to wait. To keep surgeries running as smoothly as possible, please remember the following points:

  • Please arrive on time for your appointment and allow time to park your car
  • Please let us know if you cannot keep your appointment- we will be writing to patients who do not attend appointments and they will not be allowed to pre-book appointments in future.
  • Please don’t “squeeze in” other people to your slot
  • Please let us know if you need more than the allocated 10 minutes so that we can give you a longer appointment

Requests for home visits, where applicable, should be made as early as possible in the morning.  This enables the staff and GP on call to prioritise the visits so that the more urgent ones are done first.

If you are late for your appointment the clinician will be asked if they are able to see you. If you are more than 10 minutes late you may be asked to re-book your appointment.

Weithiau, mae'n bosibl y byddai'n well gennych siarad â'r meddyg neu'r nyrs dros y ffôn yn hytrach nag ymweld â'r feddygfa am apwyntiad, a fydd yn arbed amser i chi ac i ni. Dylech roi cymaint o wybodaeth â phosibl i'r derbynnydd er mwyn i'r meddyg neu'r nyrs flaenoriaethu eich galwad ffôn. Yn anffodus, ni fydd y derbynnydd yn gallu rhoi amser neu ddyddiad penodol pan fydd y meddyg neu'r nyrs yn gallu eich ffonio'n ôl. Sylwer: Mae system ffonau'r feddygfa wedi'i ffurfweddu mewn ffordd fel nad yw gwybodaeth am y sawl sy'n ffonio ar gael a bydd yn ymddangos fel rhif anghyfarwydd ar eich sgrin. Mae rhai ffonau domestig wedi'u ffurfweddu mewn ffordd na fydd yn caniatáu galwadau o ffynhonnell anghyfarwydd. Os yw'ch ffôn wedi'i ffurfweddu fel hyn, rhowch rif arall y gall y derbynnydd eich ffonio arno.

Rydym yn defnyddio gwasanaeth neges destun i atgoffa cleifion am apwyntiadau. Dylech sicrhau bod eich rhif ffôn symudol diweddaraf gennym. Os nad ydych am i ni gysylltu â chi drwy neges destun, rhowch wybod i ni fel y gallwn godio eich cofnodion yn briodol.

Sometimes you may feel you would rather talk to the doctor or nurse on the telephone rather than visiting the surgery for an appointment, which will save both you and us time. Please give the receptionist as much information as possible in order for the doctor or nurse to prioritise your phone call. Unfortunately the receptionist will not be able to give you an exact time or day as to when the doctor or nurse will be able to return your call. 
Please note:  The surgery telephone system is set up in such a way that caller display information is not available and will appear as an unknown number on your display.  Some domestic phones are set up in a way that won't allow calls from an unknown source.  If your phone is set up like this please give the receptionist an alternative number to call you back on.

We use a text messaging service to remind patients of appointments. Please make sure we have your up to date mobile number. if you do not want to be contacted by text please let us know so that we can code your records accordingly.
