New Patient Registration

Any patient wishing to register with the Practice may book a New Patient Health check with a Health Care Assistant. The health check is an opportunity for the patient and Practice to become acquainted.
To register, a patient will be required to fill out a simple registration form giving details such as their name, address and their NHS number. To enable the medical staff to trace the patients previous medical records they will need to know the patient's previous Practice address. The GMS1 registration form can be downloaded here but needs to be brought into the surgery with the relevant identification and proof of residence.
The Practice Leaflet is available in English and Welsh and a copy can be collected from the reception desk. A larger print leaflet is available on request.
Our patient numbers have grown considerably over the last few year as have the number of patients who have moved from the Bangor and close surrounding area, but have requested to remain registered at Bron Derw. In 2013 we had to absorb patients from one local surgery closing and another closed at the end of March 2014 which has swelled our patient numbers again. In the near future we may have to look at our catchment area in consultation with the Health Board as patient care is our priority.
We appreciate some patients have requested to remain at Bron Derw and whilst we do not wish to upset anyone we would like patients to consider the distance they live from the surgery and whether being registered at a surgery closer to home would be of more benefit.
