Ymweliadau â’r Cartref   Home Visits

Doctor's bag Dylai ceisiadau am ymweliadau â chartrefi gael eu gwneud, lle bo'n bosibl, cyn 11.00 ar gyfer ymweliadau ar yr un diwrnod. Efallai y caiff ceisiadau nad ydynt yn frys sy'n dod i law ar ôl y cyfnod hwn eu hanfon ymlaen y diwrnod canlynol gan ddibynnu ar yr amgylchiadau. Rhowch gymaint o wybodaeth â phosibl i'r derbynnydd er mwyn caniatáu i'r meddyg bennu blaenoriaeth yr alwad. Cofiwch am bob ymweliad â chartref fod modd gweld saith claf, ar gyfartaledd, yn y feddygfa. Felly dylai ymweliadau â chartrefi fod ar gyfer cleifion sy'n gaeth i'r tŷ neu sydd â salwch difrifol yn unig. Yn bur anaml y bydd plant yn perthyn i'r categori hwn ac mae'n hawdd eu cludo i'r feddygfa. Byddem yn anelu at weld yr holl gleifion sâl pan fyddant yn cyrraedd y feddygfa.

Requests for home visits should be made where, possible, before 11:00 for same day visits. Non-urgent requests received after this time may be forwarded to the next day depending on the circumstances.  Please give the receptionist as much information as possible to allow the doctor to allocate priority to the call.

Remember that for every house visit seven patients, on average, can be seen at the surgery. House visits should therefore be for housebound or severely ill patients only. Children very rarely fall into this category and are easily transported to the surgery.

We would aim to see ill patients on arrival at the surgery.
